  • Last 2 years of Federal Income Tax returns (Including 1040 form and Schedule C, if self-employed and any and all 1099s and W2s)
  • Proof or evidence of Income
    • 6 months most recent pay-stubs (contact your human resources department to obtain summary page statement)
    • If your are self-employed, all documents related to income and expenses related to the business (i.e. Profit and Loss Statements)
    • Proof of any other form of income (i.e. Social Security, Retirement/Pension, Unemployment benefits, Child Support income, State Aid, Food Stamps, etc.)
    • Last 6 months bank statements on ALL bank accounts
  • Recorded Deeds, Recorded mortgages, SEVs-Property Tax Bills, and Mortgage Balance Statements for Real Estate.
  • Copies of any bill/correspondence from creditors.  Including:
    • Automobile/Car Payments (monthly statement and any letters)
    • Student loan payments /even if non-dischargeable.
    • Second mortgages and/or Home Equity Loans
    • Friend of the Court/Child support and Domestic Support Obligation(s)
      • Case Number(s) and letters received
      • Addresses of custodial parent(s)/payees
      • Statement of Arrearages owed
  • Proof of full coverage, valid Homeowners and Auto insurance showing lien-holder as loss payee (Declarations Page)
  • Summons and Complaints, Court Judgments or garnishments
  • Foreclosure notice, Correspondences from Attorney (Trott & Trott, Potestivo and Associates, Orlans, etc.)
  • Copy all vehicle contracts (i.e., Purchase Agreement or Lease Agreement) and Title(s) to all vehicles
  • Information regarding any prior bankruptcies (i.e., Michigan Bankruptcy Case number, Bankruptcy Petition, Bankruptcy Discharge Letters, Bankruptcy Notices)
